Home & Community Based
Services
Home and Community Based Services are personal
care and other services provided by our paraprofessional staff,
allowing our consumers to live safely and independently in the
comfort of their own homes. These services, funded through
government and other sources, provide consumers on-going
assistance with activities of daily living, which may include
bathing, dressing, grooming, meal preparation, housekeeping, and
errands .These supportive services can be arranged for any
frequency of time, from one to twenty-four hours of care daily,
or on a respite or temporary basis.
Adult Day Service
Addus Evergreen Clubs are adult day centers
that provide a variety of planned social, health, and supportive
services in a community-based setting. The programs are utilized
by seniors and younger adults who have physical, cognitive or
functional impairments, or may simply need companionship and can
benefit from supervised care in a safe place outside the home
during the day. The programs also afford family caregivers
respite from the demanding day-to-day responsibilities of care
giving.
Home Health Care
Home health services are provided by Nurses,
Physical, Occupational and Speech Therapists, as well as
Dietitians, Medical Social Workers, and Home Health Aides, and
can help avoid unnecessary hospitalizations, hasten hospital
discharges, and enable an individual to remain at home during
acute illnesses. Addus HealthCare provides these services
directly in the states of Indiana and Delaware. In other states,
Addus can assist consumers in selecting a Home Health Agency and
coordinate these services with the Home and Community Based
Services provided directly by Addus HealthCare, Inc.
Private Duty
Private duty home care services are the same as
those provided under the Home and Community Based program, but
are privately arranged and funded by the consumer and/or long
term care insurance. These services may be provided to persons
recovering from illness or injury, and/or as on-going assistance
with activities of daily living, and may include bathing,
dressing, grooming, meal preparation, housekeeping, and
shopping. The goal of these services is to help consumers
recover and live safely and independently in the comfort of
their own homes.
Veteran Care
Veterans and their family members may be
eligible for in-home care services, in whole or in part, through
programs and other benefits available through the Veterans
